Community Health Partnerships is part of the NHS family and we are experts in primary and community care infrastructure.  We were set up by the Department of Health and Social Care to provide high quality health and social care facilities which meet identified local need.  We work with a range of partners for the benefit of patients, service users, and those who care for them.  We maximise the benefits of our existing assets and respond to needs for new developments.  We have a unique set of skills and experience in commercial investment, community infrastructure development and property management.


Since 2001 we have enabled the transformation of health and social care services by improving the community-based estate they occupy through our NHS Local Improvement Finance Trust (LIFT) Programme.  We have developed 350 community-based LIFT healthcare facilities via 49 joint venture companies, and in partnership with the NHS and private sector partners.  Many of these are community hubs supporting the integration of local health and social care services.  


More than £2.5 billion of new capital investment has been made in new primary and community care infrastructure. The programme has contributed to the socio-economic wellbeing of UK communities; more than 40% of investment has been made in in the 10% of most deprived communities in England.  Since 2013, we have had the role of Head Tenant of 309 properties (health and wellbeing buildings) across England – 5% of the NHS property portfolio.
